Attach all required documents as listed below. Incomplete applications will not be processed. Originals must be produced at the time of admission.

Confirmation & Joining

Upon verification and seat allotment, you will receive a confirmation. Pay the fees and collect your joining kit to begin the academic year.

10th Standard Marks Card (SSLC)

Domicile Certificate / 10+2 (PUC) Marks Card

Transfer Certificate (TC) from Previous Institution

Migration Certificate (if applicable)

Eligibility Certificate for B.Sc Nursing (from INC / University)

Physical Fitness Certificate from a Registered Medical Officer

P.B.Bsc Nursing	GNM Diploma + 1 year experience. Min 45% in GNM.	2 Years
M.Sc Nursing	B.Sc Nursing — minimum 55%. 1 year experience preferred.	2 Years
D Pharmacy	10+2 with Physics, Chemistry, Bio/Maths — min 50%.	3 Years (2 + 1 internship)
B Pharmacy	10+2 with PCB or PCM — minimum 50% marks.	4 Years
DMLT	10+2 with PCB — minimum 45% marks.	3 Years (2 + 1 internship)
Bachelor of Physiotherapy (BPT)	10+2 with PCB — minimum 50%. Age 17+.	4 Years + 6 Months Internship
BSc MLT	10+2 with PCB — minimum 45% marks.	3 Years + 6 Months Internship

PCB = Physics, Chemistry, Biology. All programs are subject to seat availability and regulatory norms.
